Your health insurance carrier should provide this information in your policy. Also, your health carrier is required to give you information about your right to an external review after it initially denies payment for your claim as not being medically necessary or as being experimental/investigational. Most carriers have an internal appeals procedure, and if your claim is still denied after the internal appeals process, your carrier must again tell you about your right to external review.
